At Mind Align Psychiatry, we focus on a comprehensive approach to treat various eating disorders, including Avoidant/Restrictive Food Intake Disorder (ARFID). ARFID is a condition characterized by a significantly reduced intake of calories, not due to a desire to lose weight (as in anorexia) or a medical issue, but often due to sensory sensitivities, low hunger cues, or fear of aversive consequences.
Types of ARFID:
ARFID manifests in three primary ways:
- Sensory Sensitivity: Selective eating based on the appearance, taste, or texture of foods.
- Lower Baseline Hunger: Innate low hunger cues or those developed over time, commonly seen in individuals transitioning from anorexia.
- Fear of Aversive Consequences: Phobias affecting eating, such as emetophobia (fear of vomiting) or fear of stomach upset, leading to anxiety-driven avoidance of eating.
Prevalence of ARFID:
ARFID is seen in about 3% of the pediatric population.
Screening for ARFID:
- The Eating Disorders in Youth-Questionnaire (EDY-Q) is suitable for children aged 8-13.
- The Pica, ARFID, and Rumination Disorder Interview (PARDI) is a more comprehensive, structured interview, but requires more time.
ARFID and Risk of Anorexia:
There is a potential link between ARFID in childhood and the development of anorexia in teens. Distinguishing between the two is critical, as their dietary preferences and fears differ.
Comorbid Disorders:
Children with ARFID often have co-occurring anxiety disorders and are at a higher risk of nutritional deficiencies, including growth retardation, iron deficiency, and scurvy.
Treatment Approaches:
- Therapy: The primary treatment for ARFID involves exposure to avoided foods, often through patient-focused or family therapy.
- Resources: Online workbooks, such as those by Jennifer Thomas, Ph.D., at Harvard, provide valuable insights. Dr. Katherine Dahlsgaard is a renowned expert in this field and offers specialized care.
